General Terms: TLC Animal Hospital will exercise responsible care for the safety of your pet. We will keep the boarding premises safe and properly enclosed. Pets will be fed and given water regularly, and they will be housed in clean, safe quarters. The TLC Animal Hospital cannot guarantee against accidents, and we cannot be held liable for loss or damage caused by or to our pet guests at this facility. The owner of each pet agrees to be solely responsible for any and all attacks or damage caused by the owner's pet while he or she is at TLC Animal Hospital.
Payment/Nonpayment: The TLC Animal Hospital charges for boarding space by the day. The owner agrees to pay the rate for boarding, and this fee goes into effect on the day the pet is checked into the resort. The payment balance is due upon checking out of the hospital. If any charges are not paid when they are due, interest will accrue at 1.5% per month or the maximum rate allowed by law. All collection expenses, including attorney fees, will be paid by the owner.
Check-In/Out Times: Check-out time is by 11:00 AM. Any owner who checks out their pets before 11:00 AM will not be charged the boarding fee for the day of check-out. Any owner who checks out their pets after 11:00 AM will be charged the boarding fee for the day of check-out. Check-in and check-out times are only during normal appointment hours. There are no Sunday check-out times.
Personal Items: We make every effort possible to ensure your pet feels at ease while he or she is staying with us. The TLC Animal Hospital recommends bringing personal items from home, including beds, blankets, and toys when dropping off your pets. Please note that the TLC Animal Hospital is not responsible for lost or damaged personal items.
Vaccinations: Vaccinations are for the protection of your pet, and we cannot make exceptions to vaccination requirements. If proof of vaccination is not on file or if it is not provided by another veterinarian, your pet will be vaccinated and examined at the owner's expense on the pet's date of arrival. Your pet will have a higher risk of contracting an illness during their stay as well. There is no physical examination fee when the kennel cough vaccine alone is given.
Abandonment: If your pet is not called for within 10 days after the designated check-out time, the pet will be considered abandoned, and we will handle matters in accordance with state laws. All adoption fees and other incurred expenses will be the responsibility of the owner.
Departure Baths: We offer a departure bath for all dogs who are in our care. Please notify the representative at TLC on the date the reservation is made or during check-in if you do not want your dog to be bathed prior to departure.
Geriatric Pets: Older pets may experience additional stress when exposed to the lodging, daycare, grooming, or training environments. TLC Animal Hospital is devoted to providing exceptional care for our guests, including geriatric pets. Your signature below acknowledges that you are aware of and accept all age-related risks that your pet may face at TLC.
Medications: Medications, supplements, or other items will be administered for an additional fee as directed. However, medications must be presented in their original containers with instructions in order for them to be administered during your pet's stay.
Treatment Authorization: The owner agrees that TLC Animal Hospital, at its discretion, can give first aid, medication, or other attention we deem necessary for the health and safety of your pet. TLC Animal Hospital is authorized by the owner to provide veterinary care, including emergency care, at the owner's expense. If we believe that your pet needs care, time permitting, we will attempt to contact you before providing that care. However, this document serves as our authorization to provide veterinary care for your pet in the event we are unable to reach the owner. The owner is responsible for the expenses of veterinary care whether or not you have been reached in advance. Your signature on this authorization permits TLC Animal Hospital to make reasonable care decisions regarding your pet, and the owner agrees to pay for all costs incurred for such treatment. In the unlikely event that a pet passes away while a guest of TLC Animal Hospital, we will contact you and discuss your options for body care with you.
Monitoring: Staffing at TLC Animal Hospital varies by season, and the staff on-site depends on the day. If at any time a staff member is not present on the premises, the hospital is locked and monitored by an off-site alarm company. The pets can be observed remotely via video camera. For the safety of our staff, dogs are not walked between the hours of 10 pm and 6 am.
I hereby agree to the foregoing as the owner of the aforementioned pet. I further certify that my pet is in good health, and my pet has not been ill with any communicable conditions nor, to my knowledge, been exposed to any communicable diseases within the last 30 days. Moreover, I certify the accuracy of all information I have provided about my pet, and I have discussed any previous signs of aggression or threatening behavior toward any person or animal. I have read and understood the entire boarding contract.
